f) Procedures for Requests for Disclosure, etc. of Retained Personal Data or Records of Provision to Third Parties
- Where to Submit Requests for Disclosure, etc. Please submit requests for disclosure, etc. to the Personal Information Inquiry Desk listed above. If you wish to complete the procedures for disclosure, etc. by electromagnetic means, please inform us accordingly. In principle, we will respond in accordance with your request.
- Procedures for Requests for Disclosure, etc. After receiving your request, we will mail you the designated request form to be used. Please complete the request form and send it by post to the Personal Information Inquiry Desk above, together with documents confirming your status as representative if the request is made by a representative, and a postal money order for the fee, only in the case of requests for notification of purpose of use or disclosure. After receiving the request form, we will contact you to confirm approximately two items of personal information registered with us that can be used to verify your identity, such as your telephone number and date of birth. In principle, our response will be provided in writing by sealed mail directly to the individual.
- Documents to Confirm Representative Status When the Request Is Made by a Representative If the person making a request for disclosure, etc. is a representative, please enclose documents proving representative status and documents verifying the identity of the representative. Any information concerning registered domicile contained in such documents should be limited to the prefecture; any information beyond the prefecture should be blacked out or otherwise redacted. Please also submit documents that do not contain individual numbers, or redact all digits of any individual numbers by blacking them out or using other appropriate methods.

- Establishment of Basic Policy To ensure the proper handling of retained personal data, we have established a Personal Information Protection Policy covering matters such as compliance with relevant laws, regulations, and guidelines, as well as a contact desk for questions and complaints.
- Establishment of Rules for the Handling of Retained Personal Data For each stage of handling retained personal data, including acquisition, use, storage, provision, deletion, and disposal, we have established personal information protection regulations concerning handling methods, responsible persons, personnel in charge, and their duties.
- Organizational Security Control Measures
- We have appointed a person responsible for the handling of retained personal data, clarified the employees who handle retained personal data and the scope of retained personal data handled by such employees, and established a reporting and communication system to the responsible person in the event that facts or indications of violations of laws or handling regulations are identified.
- We also conduct regular self-inspections of the handling status of retained personal data and conduct audits by other departments and external parties.
- Human Security Control Measures
- We conduct regular training for employees on important points concerning the handling of retained personal data.
- We obtain written confidentiality pledges concerning retained personal data and other confidential information from all employees.
- Physical Security Control Measures
- In areas where retained personal data is handled, we manage employee entry and exit, restrict devices and other items brought into such areas, and take measures to prevent unauthorized persons from viewing retained personal data.
- We take measures to prevent theft or loss of devices, electronic media, documents, and other materials that handle retained personal data. When such devices, electronic media, or similar items are carried, including movement within our business premises, we take measures to ensure that retained personal data cannot be easily identified.
- Technical Security Control Measures
- We implement access controls to limit the personnel in charge and the scope of personal information databases and similar resources handled.
- We have introduced mechanisms to protect information systems that handle retained personal data from unauthorized external access and malicious software.
